In December 2014 a collaboration with Hepburn Health Service, Hepburn Shire Council, and community members hosted a forum called ‘Stand by Me’.
The forum focused on the needs and nature of care-giving for ageing family and friends.
This was the forerunner to the next project, Ageing Well in Hepburn Shire. With this project we hoped to foster positive approaches to ageing and to care-giving by providing essential information for living well in the later stages of life, including how to access local resources as we need them.
A series of talks on ageing well and care-giving in Hepburn Shire, ‘The Ageing Well Talk Series,’ was held in 2015.
Following that, a series of three forums was held on 'Dying Well in Hepburn Shire' - in Daylesford, Clunes and Creswick (plus a slightly different talk in Trentham via the community education day planned by the Trentham Branch of the ALP). The forums were supported by a grant from Grampians Region Palliative Care Consortium.
One product of these conversations has been the 'End of Life Paperwork' document to help people become better prepared for the practical aspects of the end of life period. You are most welcome to have a copy of this, found here.
